Kojima releases a brand-new Metal Gear Solid 5 trailer that gives a little info on the story.
Revenge. That's the point. Big Boss has last everything, but his enemies made one BIG mistake: they didn't the world's greatest soldier out. They left him sleeping in a haunted hospital. When he awakes, the poor souls who didn't finish the job are going to wish they never messed with Big Boss.
This new E3 2014 trailer is not big on coherence (none of them really have been, as they leave us to parse the pieces together for some notion of the plot), but it does a good job of showing us that Big Boss is out for blood.

I can only assume that Big Boss and Kaz rebuild Outer Heaven, with the help of Ocelot, and rebrand themselves as Diamond Dogs -- something about making diamonds out of their fallen comrades' ashes? I don't know...
We see a lot of training in progess in the sections of the trailer. Ocelot and Snake teach their troops CQC, while Kaz plots their revenge. And poor Huey? Well, it looks like he's going to get interrogated by Ocelot, which is a bad sign. If there's one thing we know about old Revolver, it's that the man is ruthless when it comes to torturing people for information. Did Huey betray his comrades during the events of Ground Zeroes? We'll soon find out.
Two things that caught my attention: 1) all the masked men, especially the one that wants to shoot from the hip during target practice reminds me of Johnny, who tends to get himself into a lot of trouble. Of course, he'd be too young to participate in Metal Gear Solid 5, so maybe it's his dad? Kojima loves father/son roots. 2) Big Boss is hanging out with Skull Face, the assumed villain of MGS5?! Have Skull Face and Big Boss teamed up against their common enemy? We know that at Skull Face is looking for Major Zero. Maybe Big Boss has decided to team up with him...Also, METAL GEAR?!
